Healthcare information

The public inquiry form is not intended for patient information or protected health information. Do not submit medical records, patient names, dates of birth, diagnoses, insurance identifiers, or other sensitive clinical information through the website.

Retention and security

JSC uses reasonable administrative and technical measures to protect inquiry information. No internet transmission or storage system can be guaranteed to be completely secure. Information is retained only as reasonably necessary for business and legal purposes.
